1.1. Medical prescription
You will need a doctor’s prescription for your treatment. You will receive this from the doctor you trust, who is authorized to issue this prescription. In addition to personal data, the prescription must include
• a medical diagnosis
• the number of treatment units and
• the prescribed treatment

The requirement for a doctor’s prescription can only be waived if you use the services of your physiotherapist exclusively for preventive purposes. According to professional law, preventive services may only be provided to healthy persons. If you are suffering from pain, for example, or if you know of or have other conditions that require treatment, please inform your physiotherapist immediately.
1.2. settlement of the treatment costs
The cost of the treatment is based on a combination of the individual service, the time required and any materials needed for the treatment and will be charged to you at the time of treatment.announced at the beginning of the session. Your physiotherapist does not have a contract with your health insurance carrier. You settle the costs with your treating physiotherapist as an optional therapist and apply to your competent health insurance provider for partial reimbursement in accordance with the cash tariff/statutory cost allowance. Information on the expected reimbursement/cost subsidy can only be given subject to the decision of your social insurance carrier.
1.3. Approval from your health insurance provider
Your health insurance carrier will cover part of the treatment costs. For this purpose, you need an approval of the medical prescription by the chief medical department of your responsible health insurance company. In this way, the health insurance provider approves the reimbursement of the proportional costs/the statutory cost allowance after the treatment has been carried out and after the treatment costs have been paid on the basis of the fee invoice submitted at the same time as the prescription approved by the chief physician.
1.4. findings
Professional treatment requires a detailed initial assessment. Your physiotherapist is dependent on your assistance in this process. Therefore, you are asked to bring all relevant findings to the first appointment.
2. what is the course of the therapy?
2.1. Personal one-on-one care
Your physiotherapist is exclusively at your disposal for the duration of the treatment. She/he is your contact person for organizational and technical questions regarding the treatment. With her/him you agree on the areas that are important for you, such as …

2.2. Your treatment
The services provided by your physiotherapist include all measures performed directly with and for you, in particular
- personal individual treatment including diagnosis and consultation
- treatment-related administration, scheduling of appointments
- Preparation and follow-up necessary for the treatment, e.g. production, adaptation and provision of individual therapy materials
- Documentation (medical history) and 10-year retention, whereby you have a right to inspect and copy (against reimbursement)
- if required/on request: compilation of individual findings going beyond the documentation for submission to various bodies such as health insurance carriers, treating physicians, private insurance carriers and similar bodies
With your signature following a treatment session, you confirm the utilization of the treatment. This is a requirement for your health insurance carrier to cover the costs.
2.3. Principles of treatment of your physiotherapist

Treatment is provided in accordance with the relevant legal provisions, in particular the Federal Law on the Regulation of Senior Medical-Technical Services, as amended. (MTD Act) - Science:
Your physiotherapist is guided by the latest scientific findings. - Self-determination:
Your physiotherapist will make a treatment proposal based on the doctor’s prescription and the initial assessment. It is up to you to accept this offer or to discuss adjustments with your physical therapist. - Secrecy:
All information you give to your physiotherapist is subject to confidentiality. It is assumed that an exchange of information for the purpose of optimizing treatment is desired with the prescribing physician as well as with the other health care professionals named by you and involved in the treatment. This information will not be disclosed to any other person without your consent. If further disclosure of information proves useful and necessary for medical-therapeutic reasons, your physiotherapist will consult with you about it. The same applies to the disclosure of documentation that is obligatory for legal reasons.
2.4. Documentation
Your physical therapist is required by law to document, among other things, therapeutic interventions in a medical record. The documentation is the property of your physical therapist. At your request, you may inspect the documentation and obtain copies at cost. After completion of the treatment, the documentation remains with your physiotherapist and is kept for the legally obligatory period of 10 years.
3. what should you know about the cost of treatment?
3.1. Amount of the costs
The cost is based on a combination of the individual service, the time required and any materials needed for the treatment. Please refer to your physical therapist’s enclosed fee schedule for a representation of cost centers upon which this agreement is based. The costs of the individually envisaged treatment will be communicated to you by your physiotherapist at the beginning of the treatment and are based on the enclosed fee schedule of XX.XX.2019.
3.2. Payment mode
At the end of the treatment (or treatment sessions of the doctor’s prescription), your physiotherapist will issue you a fee note for the total cost of the treatment sessions.
You can arrange the following payment method with your physiotherapist:
– Cash payment
– Payment with payment slip/bank transfer
You agree with your physiotherapist on the date of payment (due date) – it can be found on the fee invoice. If you are in default with the agreed payment method, your physical therapist reserves the right to charge interest on arrears at the legally permissible rate of 4%. For reminders issued in connection with fee claims not paid according to the due date, the reminder fees charged shall amount to 5 euros for the first reminder and 10 euros for the second reminder. Euroand for the third reminder to 20 Euro,-. The total cost of the treatment is therefore the fee plus any interest on arrears and reminder charges incurred.
4. what is your part in a successful treatment?
Your physiotherapist is a companion on your very personal path and is there for you with advice and support. During the initial assessment, treatment goals and measures are discussed and agreed upon. Successful treatment requires that you provide your physiotherapist with information about your state of health and the examinations and treatments that are related to your current complaints and that you have undergone so far. Your physiotherapist will support you by asking specific questions.
In order to achieve the best possible treatment success, your assistance is indispensable. Assistance may mean following certain instructions for action, repeating learned exercises, or refraining from certain actions.
If your physiotherapist gets the impression that the success of the treatment does not seem achievable, e.g. due to a lack of your cooperation, your physiotherapist will address you about it and try to offer a solution.
5. how do you cancel an agreed treatment appointment?
If you are unable to attend an appointment, you are requested to inform your physiotherapist immediately, but no later than 12 hours before the scheduled appointment (or no later than one day before the opening of the practice). Otherwise, your physiotherapist reserves the right to charge you for the unattended appointment in the amount that you would have had to pay even if the treatment had been performed. (or also: “in the amount of a fixed amount of 2/3 of the individual treatment costs,-“). These costs cannot be claimed from the health insurance carrier.
6. when does the treatment end?
The doctor’s prescription limits the scope of treatment. If treatment beyond this is necessary, you will need a new prescription from a physician (if you are seeking reimbursement, also approved by the chief physician).
The treatment usually ends in agreement between you and your physiotherapist. Both you and your physiotherapist are also free to discontinue treatment at any time without giving any reason. In particular, your physiotherapist will decide to discontinue the treatment if he/she is of the opinion that the treatment will not lead to the desired or agreed success or if other medical-therapeutic treatment measures are indicated.
The same applies if, for example, your physiotherapist no longer considers the treatment to be responsible from a therapeutic point of view, or if you do not comply with the agreed mode of payment. In the event of premature termination, the treatment sessions that you have actually used will be charged. An exception are appointments that are not canceled in time (see above).
7. how do you apply to your health insurance carrier for reimbursement of treatment costs according to the tariff/statutory cost allowance?
You submit the medical prescription approved by the chief physician before the start of the treatment – provided withwith the data of the treatments already carried out signed by you – and the fee note issued by your physiotherapist to your health insurance provider and request a transfer to an account specified by you or a postal order of the amount to be reimbursed according to the cash tariff/statutes of the social insurance provider. Your physical therapist will advise you on the approximate amount your health insurance will reimburse/subsidize. Information on the expected cost reimbursement/cost allowance can only be given subject to the decision of your social insurance agency.
